What can the Pharmacist Program do for me?
The goal is simple: assess, review and make recommendations regarding your medication management.
How Does the RDPCN Pharmacist Program work?
Your family doctor will send a referral to your PCN pharmacist assigned to their clinic, if he/she feels you could benefit from a medication review.
Initial Appointment:
You will receive a phone call from your PCN pharmacist to arrange an initial meeting at your family doctors office, or in your home. This is an opportunity to:
- Get to know the pharmacist
- Explore your needs
- Review your medication and make recommendations that will guide you in successful medication management.
- After your appointment your pharmacist will send your family doctor a summary of the meeting. They will together with you to adjust your medications as necessary.
